  • Age: 15+
  • Medication: Inadequate response to 2 antidepressants from different medication classes, taken at any point in your treatment history. Each must have been prescribed at the maximum dose for at least 6 weeks, unless intolerable side effects.
  • Therapy: Inadequate response to evidence-based psychotherapy
  • Depression screening: PHQ-9 score of 15+ within the past 4 weeks

HMSA, BCBS, UHA, HMSA Quest & VA CCN

  • Diagnosis: Major Depressive Disorder (MDD)
  • Age: 18+
  • Medication: Inadequate response to 2 antidepressants from different medication classes within the past year. Each must have been prescribed at the maximum dose for at least 6 weeks, unless intolerable side effects.
  • Therapy: Inadequate response to evidence-based psychotherapy
  • Substance use: Evaluation showing no risk for a substance use disorder. If a disorder is present, documented remission with 6 months of complete abstinence and a urine drug screen within the past 3 months are required.
  • Depression screening: PHQ-9 score of 20+ within the past 2 weeks
